Simply complete the enrollment form and submit it by mail,  email to enroll@creativejourneys-cclc.com or fax it to 410-676-9003 or in person and we will contact you to finalize your enrollment!

Before you begin be sure you have the following:

• Your child’s most recent immunization records (Request from Doctor)
• The names and phone numbers for emergency contacts

If you have any problems using the enrollment form, please contact our Center Director at director@creativejourneys-cclc.com.

• Applications are accepted year round on a first come, first served basis.
• Send Medical Immunization Records with your application for each child.
• Please return your application as soon as possible to secure slot availability.
• $50 Registration Fee required to complete admissions application.
